This is for callers reporting animal welfare concerns, dangerous animal-related situations, as well as crises or questions at pet resorts, veterinary facilities, farms and other animal-related facilities.

• What kind of animal(s) are involved and how many? (dog, cat, horses, cows, etc.)  

• Where are the affected animals located? 

• Is it public or private property where the animals are located? (Who owns the property?) 

• (If the caller is law enforcement) What is the Event Number?
